To expedite your first visit, please download and complete the patient questionnaire and privacy practices (HIPAA) form prior to your scheduled appointment and present them to the receptionist along with your insurance card(s) and drivers license.
These are the items necessary for your first visit:
- Complete paperwork
- Insurance Card
- Picture I.D./Drivers License
- All radiological films pertaining to your visit with Dr. Keem (Actual films or Disc, they CANNOT be faxed)
- The written report that goes along with your radiology films
- Referral to our office, if your insurance requires one
- Copay
- List of ALL medications and dosages
